Welfare Plan

EXTENSION OF MAJOR MEDICAL AND DENTAL BENEFITS ON AN EMPLOYEE'S DEATH

If you die while an active Employee, your eligible dependents will continue to be covered for Major Medical and Dental Benefits for 6 months at no cost. At the end of 6 months, continuation of such benefits, subject to some restrictions, will be available at the currently set cost of $50 per month, plus applicable taxes.

This rate is reviewed from time to time by the Board of Trustees and may change in the future. Payments for coverage must be received by the Plan Administrator prior to the end of the month for which they are due and failure to make any payment when due will result in cancellation of this optional coverage.

The benefits outlined under the Plans are subject to the terms and conditions of the Plan documents and Group Master Policies. If there is any conflict between this outline and the Plan documents and Group Master Policies, the Plan documents and Group Master Policies will apply in all cases. Also remember that no benefits are guaranteed and that the benefits can be changed by the Trustees at any time.
