Am I covered for Benefits under the Welfare Plan yet?
You require 900 hours within 9 months to initiate coverage.

I didn`t receive my Pay Direct Drug Cards yet?
Before new Pay Direct Drug Cards are issued, you must complete a member information card and a Co-ordination of Benefits form.  See also: Welfare Plan Forms

Why is my Pay Direct Drug Card not working?
Check to make sure your pharmacist is entering all your information correctly.  Is your name or your dependents` names spelled correctly?  Is the date of birth accurate? etc.

 

Existing Members

Why is the name of my Pension Beneficiary incorrect?
If you had a previous spouse and we do not have a copy of your separation or divorce agreement on file, your previous spouse must remain on our system as your beneficiary.  See also: Welfare Plan Forms

What Year is the Dental Fee Guide on for our Plan?
Eligible dental benefits will be reimbursed in accordance with the O.D.A. Fee Schedule (current minus two years) for all provinces.  For example: 2012 O.D.A. applies in 2014 calendar year.

What percentage is basic dental work reimbursed at and what percentage are major dental procedures reimbursed at?
Eligible basic dental is reimbursed at 100% of the applicable O.D.A. Fee Schedule and eligible major dental is reimbursed at 80% of the applicable O.D.A.

How often are recall examinations by the dentist covered under our plan?

Can I use my Pay Direct Drug Card for the payment of my Vision Care or any other medical supplies?
No.  Your Pay Direct Drug Card is only to be used for your prescription drugs.  You should continue to submit claims to Manion Wilkins & Associates for all other medical and dental claims.  See also: Welfare Plan Forms

How can I claim for prescription glasses or contact lenses?
Vision Care benefits are covered once every two calendar years to a maximum of $400.
